Existing students

Requirements:

  • Complete clearance from previous sem or school year
  • Copy of grades

New students

Requirements:

  • Form 138/Report Card & Diploma
  • Certificate of Good Moral Conduct
  • PSA Birth Certificate & 2x2 Picture

Transfer students

Requirements:

  • Transcript of Records (TOR) or Certificate of Grades
  • Honorable Dismissal & Good Moral
